Lack of courage from the leadership which is risk averse and want to protect the status-quo.
The mental model that fail is a lack of competence and fear of punishiment.
A second point is the willingness to ramain inside the confort zone (not be vulnerable and defesiveness of their position of power). Fear of the new, because the organizations rewards execution and fast success.
